How Do I Apply?

The underwriting process at AMS involves more physician participation than many other insurance companies require. Be assured that all AMS colleagues are subject to the same standards. The process is as follows:

  1. Request an application through Our Representative either by calling 866-461-1221, or sending an email to info@amsrrg.com.

  2. An application is then sent, with all the necessary supporting documents.

  3. The completed application should be sent to:

    Questions may be directed to Our Representative, either by email or phone 866-461-1221 . (Note: the underwriting process can only be finalized upon submission of a completed application).

  4. The selected physician is then sent the premium quotation, including the Offering Circular and supporting information.

  5. Upon submission of all signed documents and the first premium payment, the policy commences on its effective date.
