If you have been notified about an impending legal action, or you want to notify AMS RRG about the potential of an action, please call 866-520-6896 or 469-330-6355 to speak directly with our Claims Representative.

Be prepared to describe what documents you have received, including:

• Patient complaint letter
• Notice of Intent
• Summons and Complaint
• Subpoena for a deposition
• Any occurrence that you feel should be reported.

You will need two (2) copies of your Certificate of Insurance, all pertinent records, the Notice that you received, and any other information that you feel is pertinent to the matter.

Keep an original and 1 copy together in a secure place.
